Please complete the application below to be considered for support through Crowned With Purpose, our initiative providing medical wigs and hair restoration support to breast cancer patients and survivors in our community. Eligibility 

​

• Female, age 18 or older 

• Current or previous diagnosis of breast cancer

• Experiencing treatment-related hair loss, thinning, or medically significant regrowth challenges

• Resident of Kern County OR currently receiving breast cancer treatment in Kern County

• Willing to complete a consultation with an approved provider

• Has not previously received assistance through Crowned With Purpose

• Demonstrates financial need

• Agrees that payment is made directly to the provider
